Medical Certificate
All transplant (recipient) competitors need to have the Medical Certificate below completed, signed and stamped by their Transplant Physician (not their GP). The medical certificate cannot be completed before August 2012. Certificates dated prior to August 2012 will not be accepted.

The Organising Committee reserves the right to spot check the authenticity of the medical certificate by contacting the Transplant Physician.
Waiver
Everyone who registers (competitors, supporters and volunteers) must also complete and return the Waiver and Release Form below.

Both the Medical Certificate and Waiver forms must reach Transplant Australia’s National Office by the 24th August 2012.

Dialysis Patients
Dialysis patients should organise their own treatment in the same way they would normally do when taking a holiday.
Insurance cover during the Games
All registered participants will be covered whilst attending the Games by the Sports Insurance Policy that has been taken out by Transplant Australia Ltd.
Travel Insurance for recipients and family members
A comprehensive travel insurance policy that includes medical cover for both recipients and accompanying persons is now available through Chartis Travel Insurance.
Transplant members will receive a 30% discount on the normal travel insurance cost but there is a one-up assessment fee (only for recipients) that receive approval from Chartis for travel insurance. Details of this will be provided by Chartis when applying for the insurance.
To obtain the insurance you will need to quote your membership number. You can obtain this by logging in to http://www.transplant.org.au/ and clicking on membership.
Once your policy is in place it can also be used for any other travel undertaken by financial Transplant Australia members.
