Medical Certificate
All Transplant Recipients, competing at the 13th Australian Transplant Games in Newcastle are required to obtain a valid medical certificate issued and stamped by their Transplant Specialist (not GP), that cannot be dated before 1st August 2012 and must be returned to Transplant Australia no later than Friday 31st August 2012. The medical form can be downloaded below.
Important: Please contact your specialist NOW to make an appointment to ensure you can be seen in that time period.

The Organising Committee reserves the right to spot check the authenticity of the Medical Certificate by contacting the Transplant Specialist.
Waiver
Everyone who registers (competitors, supporters and volunteers) must also complete and return the Waiver and Release Form below.
General Waiver 2012
Both the Medical Certificate and Waiver forms must reach Transplant Australia’s National Office by the 31st August 2012.

Dialysis Patients
Dialysis patients should organise their own treatment in the same way they would normally do when taking a holiday.
To arrange for dialys in the Public Sector contact Leanne Avis on 0403 368 284. Leanne is the Dialysis Coordinator at the John Hunter Hospital.
Insurance cover during the Games
All registered participants will be covered whilst attending the Games under the Public Liabilty and Sports Insurance Policy that has been taken out by Transplant Australia Ltd. This does not cover travel, accommodation and medical or personal costs.
Travel Insurance for recipients and family members
A comprehensive travel insurance policy that includes medical cover for both recipients and accompanying persons is now available through Chartis Travel Insurance.
Transplant members will receive a 30% discount on the normal travel insurance cost but there is a one-up assessment fee (only for recipients) that receive approval from Chartis for travel insurance. Details of this will be provided by Chartis when applying for the insurance.
To obtain the insurance you will need to quote your membership number. You can obtain this by logging in to http://www.transplant.org.au/ and clicking on membership.
Once your policy is in place it can also be used for any other travel undertaken by financial Transplant Australia members.
