Medical Certificate and Waiver:

 

Medical Certificate: All Transplant (Recipient) Competitors need to complete and return the Medical Certificate below. The medical certificate from your doctor must be dated in August  2010.  Older certicates can not be accepted.

Waiver:  All registrants (competitors, volunteers and supporters) need to complete and return the Waiver and Release Form below.

Both form must reach the Translant Office by the 27th August 2010.

Dialysis Patients

Dialysis patients should organise their own treatment in the same way they would normally do when taking a holiday. There are a number of dialysis centres in Canberra including:

Insurance

All registered participants will be covered by the Sports Insurance Policy that has been taken out by Transplant Australia Ltd. For full details please see the policy below.

Travel Insurance for Recipients and Family Members

We are pleased to advise that we have negotiating a comprehensive travel insurance policy to cover both recipients and accompanying persons.

As part of the negotiations we have secured a 30% discount for our members but this will only be available to Financial and Honorary Members, so make sure that you have updated your status before applying for travel insurance.  To obtain the insurance you will need to quote your new Membership ID. You can ontain this by logging in to www.transplant.org.au and click on membership.  You will be asked for your login and password and after that you can click on the "Travel Insurance" option.  Your login will be your current email address or "your membership ID@transplant i.e. 234@transplant.  The password will have been emailed or mailed to you and should be changed when you first logged in using this new option..

Once your policy is in place it can also be used for any other travel undertaken by financial Transplant Australia members.
